Professional Therapeutic Tier 1

Online Membership Registration

- Enter the names to include on the membership as cardholders. Please enter the primary member’s information first, then any additional adults that your member level allows.
- Memberships are non-refundable, non-transferable and cannot be used for group visits of any kind.
- Membership prices and benefits are strictly for named cardholders and are subject to change at any time, without prior notification.
- A photo ID is required at time of admission for entrance.
